Before your appointment

After you register, you will receive a packet in the mail containing a welcome letter confirming your appointment, a health history questionnaire and a list of things to do before you come in for your appointment.

Bring to your appointment:

• The completed appropriate health questionnaire you received in the mail or download it here:
  • General Health History Questionnaire
  • Lymphoma Patient Health History Questionnaire
  • Dermatology Patient Health History Questionnaire
     
• A list of your current medications or download a form here:
  • The Med Form
     
• Your diagnostic films, for example X-ray scans, etc.

Before your appointment, contact your current physicians and have them fax your records to (520) 694-0255.
